Effect of Intradialytic Isometric Exercise with or without Neuromuscular Electrical Stimulation on Tumor Necrosis Factor Alpha in Chronic Kidney Disease Patients

Abstract

Most chronic kidney disease patients on hemodialysis have muscle wasting. Increase of Tumor Necrosis Factor Alpha (TNF-α) in chronic renal disease is a factor that causes muscle wasting via the NFkB/ubiquitine proteasome pathway. Strengthening exercises recommended by the ACSM are hard to do, so the adding of Neuromuscular electrical stimulation (NMES) may be needed to facilitate muscle contraction. Contraction of skeletal muscle will express IL-15 which is important for metabolism and skeletal muscle function and also inhibits TNF receptors. Besides this, muscle contraction will increase IL-6 which increases soluble TNF-α receptors, so TNF-α level will decrease. Isometric exercises and NMES are easy to be applied intradialytically and can facilitate muscle contraction, but their effect in chronic kidney disease patients has not been largely reported yet. This study aimed to prove that intradialytic isometric exercises with or without NMES can modulate TNF-α expression in chronic kidney disease patients. This experimental study had a pretest-posttest control group design. Twenty-five chronic kidney disease patients on hemodialysis were randomly divided into 3 groups: control group, isometric exercises group, and isometric exercises and NMES group, which received treatment twice a week, for 6 weeks. The subjects were examined for TNF-α test by immunoassay pre and post study. There was decreased TNF-α pre experimental compared to post experimental in the isometric exercises group (p=0.003) and isometric exercises and NMES group (p=0.025). TNF-α changed significantly in the isometric exercises group (p=0.017) and isometric exercises and NMES group (p=0.020) compared to the control group. Intradialytic isometric exercises with or without NMES can regulate TNF-α in chronic kidney disease patients.
