Rehabilitation Programs for Total Hip Arthroplasty in Patients with Enclosed Hip Bilateral: A Case Report

Abstract

Bilateral hip ankylosis is a rare entity. It is a chronic inflammatory disorder in the musculoskeletal system. Progressive complaints of decreased hip mobility supports the patients’ difficulty in walking. Choice of management including Total Hip Arthroplasty and rehabilitation intervention is needed for a better outcome. The case study is of 24-year-old male with a decrease of bilateral hip and trunk mobility diagnosed with bilateral hip ankylosis and ankylosing spondylitis. He had difficulty with mobility, ambulation and a decrease in respiratory capacity when doing daily activity. He also had done a bilateral total hip arthroplasty. Our rehabilitation goals were to improve hip mobility, gait pattern, to prevent joint stiffness, maintain respiratory capacity and independent functions. The management of bilateral hip ankylosis is difficult to treat and requires effective pre- and post-surgery rehabilitation. The progressive and chronic inflammatory will decrease the bilateral hip mobility, generates deformity and decreases independence functions. Our rehabilitation programs includes a range of motion exercises, muscle strengthening exercises of the lower extremities, gait training exercises, breathing exercises and endurance exercises. After rehabilitation program, the patient showed improvements in joint mobility, ambulation, respiratory capacity function and did more activities for daily living independently. Effective rehabilitation program pre- and post-surgery could improve the life of patients with total hip arthroplasty due to bilateral hip ankylosis in becoming more active, mobile and living independently.
