Abstract:
Adolescent idiopathic scoliosis reduces the quality of life, is painful, increases cosmetic deformity, and involves cardiopulmonary problems. Scoliosis coincidence with tuberculosis destroyed lung is rare. We give conservative management of adolescent scoliosis with a Cobb’s angle of more than 450 in patients with tuberculosis destroyed lung who refuse surgery. This case reports the conservative management of adolescent scoliosis with a Cobb’s angle of more than 450 in a patient with tuberculosis destroyed lung who refused surgery treatment. A 15-year-old girl consulted our department with back pain and a skewed back. She felt shortness of breath following sport and climbing stairs. Physical examination revealed that she had double curve scoliosis. A thoracic curve was formed by VTh 3 and VT 11 with apex VTh 7. Cobb’s angle was 530. Lumbal curve was formed by VTh 11 and VL 5 with apex VL 1. Cobb’s angle was 400. Moe was 2, Risser was 3. She refused surgery. She had a history of MDR T
B with tuberculosis destroyed lung on medication. Tuberculosis medication was etambutol, levofloxacin, pyrazinamid, etionamid, cycloserin and multivitamins. PM&R programs were scoliosis exercises, breathing exercises, breathing control and chest expansion exercises, endurance exercises, postur correction and wearing a TLSO (milwaukee brace). After the scoliosis conservative rehablitation program for one year, the patient showed positive improvement in her level of back pain, Cobb’s angle degree progression, and cerdiorespirasi endurance. The scoliosis conservative rehablitation program can be considered as a therapy in adolescent scoliosis with a cobb’s angle of more than 450 in patients with tuberculosis destroyed lung who refuse surgery
