Abstract

Denpasar is the first city to implement a TB screening program among people with diabetes mellitus (DM). This study was aimed at exploring possible obstacles and solutions for implementation of TB screening among people with DM in Denpasar, Bali, Indonesia. This was a needs assessment to begin screening program through consultations, group discussions, and workshops. The important issues were the X-ray coverage which is for all patients to each have a chest X-ray. There are two main solutions; firstly, the algorithm is modified from the original in that all patients should go through a chest X-ray after anamnesis in following two groups: 1) patients with a productive cough go straight to the sputum smear test, 2) patients with other/no symptoms proceed to X-ray. The second result was the agreement made to meet BPJS regulation in order to avoid increasing referral burdens for PHCs. Therefore, we include DM patients who have been diagnosed for at least 3 months. A screening algorithm may need some adaptation to meet field situation and viewpoints of all stakeholders. Moreover, the involvement of all related stakeholders and negotiations to meet their expectation without undermining purpose of program will increase the feasibility to put program in place.
