Genotype Hepatitis B Virus Among Intravenous Drug Users with Occult Hepatitis B Infection in Surabaya, Indonesia

Abstract

Abstract: Worldwide, an estimated 400 million people are infected with HBV, and Southeast Asia, specially Indonesia, has a high endemicity level of HBV infection. The virus is transmitted by parenteral exposure to infected blood or body fluids. Intravenous drug users comprise a high-risk group. According to the National Narcotics Board of Indonesia (BNNRI), especially East Java, the drug abuse cases rise every year. In practice, the infection of HBV is diagnosed by ELISA to detect hepatitis B surface antigens (HBsAg). However, ELISA does still have the potential to yield false negative reactions because of the possibility of non-specific reactions in ELISA tests for HBsAg. The aim of this study was to determine the genotype of HBV in specimen sera among intravenous drug users with HBsAg negative in Surabaya, Indonesia. Seven sera (7.1%) had positive HBV DNA from 99 patients with HBsAg negative. All the samples were investigated for HBV DNA using Polymerase Chain Reaction (PCR) with pairs of primers (P7 and P8; HBS1 and HBS2) based on the surface HBV gene. The DNA positive samples were sequenced and analyzed to determine the genotype of HBV using the computer program Genetic Version 9. Genotype B was detected in all positive HBV DNAs (100%). It is likely that genotype B HBV is predominant in Indonesia. It needs further investigation to explore hepatitis B prevalence by molecular detection and genotype distribution with a greater number of participants who areHBsAg negative.
