Urinary Incontinence Prevalence and Its Relation between Pelvic Floor Muscle Strength and Type of Incontinence Urine Based on Incontinence Questions (3IQ) among Elderly Men

Abstract

Background: The prevalence of urinary incontinence (UI) increases with age. Urinary incontinence occurs in 11 to 34 percent of older men. Two to 11 percent of older men report daily urinary incontinence. Pelvic floor muscle (PFM) weakness is one of the conditions which cause UI in elderly men. Material and Methods: Twenty-five elderly men participated in this study. An interview Urinary Incontinence questionnaire was adapted from 3 Incontinence Questions (3IQ) as outcomes reported by the patients. Manual muscle testing (MMT) as outcomes informed by the clinician were assessed. Result: About 25 completed the study.  The overall Urinary Incontinence prevalence from 3IQ was 52%, which consisted of prevalence of stress UI (23.1%) with MMT <3 (100%), urgency UI (38.4%) with MMT >3 (100%), functional UI (23.1%) with MMT >3 (100%) due to osteoarthritis (33.3%) and due to post stroke (66.6%), mixed UI (15.4%) with MMT <3 (100%). Other data showed the result of mismatch between subjective and objective data (48%). They didn’t complain of UI but the MMT was <3 (50%) and MMT >3 (50%). Anova analysis showed no relation between 3IQ and MMT (r=0.189; P=0.05), and 13 positive incontinence subjects were analyzed with Anova analysis. An influence correlation between MMT and type of UI (r=0.033; P=0.05) was found. Functional UI (r=0,009), Urge UI (r=0,017), stress UI (r=0,396), mixed UI (r=0,274. Conclusion: The high prevalence of UI in elderly men is similar to the previous study that showed UI prevalence especially urge UI which becomes more common with increasing age. Patients with stress UI have weak PFM. Patients with urgency UI have good PFM. Patients with functional UI have good PFM. Patients with mixed UI have good and poor PFM. The mismatch between subjective and objective data may be due to shame or the patient's ignorance about UI
