Anal Pressure Biofeedback Training in a Patient with Diverting Colostomy after Blunt and Penetrating Anorectal Injury: A Case Report

Abstract

Pelvic floor muscle tone weakness especially in the external anal sphincter is often related to colostomy procedures. Study shows that temporary stoma lead to morphological changes of the anal sphincter. The sphincter anal muscle undergoes hypotrophic malformation. It leads to difficulty in bowel opening, even after stoma closure. A 13-year-old boy consulted our department with a penetrating anorectal injury after falling into a sitting position and his anorectal was hit by a paralone. The patient underwent a diverting colostomy and had a stoma in the left abdomen. Physical examination revealed that he had a weak external sphincter anal muscle contraction. The physical medicine and rehabilitation (PM&R) programs consist of anal pressure biofeedback training three times a week, pelvic floor exercises at home three times a day and endurance exercises with walking 30 minutes every day. After serial anal pressure biofeedback training, seven times for three weeks, there were improvement in minimal anal sphincter contraction (34 to 27), maximal contraction (57 to 180), average contraction (42 to 66), and power contraction (10024 to 15927). Anal pressure biofeedback training gave improvement to anal sphincter contraction in patients with temporary stoma. Pressure anal biofeedback training can be considered as an adjunctive therapy to help restore proper anorectal function before stoma closure.
