Rehabilitation Management in Late-Findings of Ankylosing Spondylitis Patient: A Case Report

Abstract

Introduction: Case findings of Ankylosing Spondylitis (AS) patient over 50 years old are uncommon. Identification of clinical presentations and radiograph are required for diagnoses. These findings may influence the rehabilitation management.Case Presentation: A 56-year-old man visited the rehabilitation outpatient clinic in June 2017 for stiffness of his neck and back, severe neck pain (VAS 8-9) and functional limitations. He was obese and diagnosed with Coronary Artery Disease and Diabetes Mellitus Type 2. Clinical presentation revealed an abnormal posture; ROM limitation of several joints; para cervical and upper trapezius muscle spasm; decrease of trunk flexibility, chest expansion and energy expenditure. Plain radiographs revealed a bamboo spine appearance. We gave him rehabilitation management for 20 sessions, two times a week, and we achieved improvement of physical and functional abilities. Some patients show improvement of their psychological conditions. Discussion: Late-findings of AS patients over 50 years old are caused by underdiagnosis. It is important to make the patients understand about their condition and give them appropriate rehabilitation management based on evidence and consensus recommendation to support their improvement.Conclusion: Earlier findings to make a diagnosis of ankylosing spondylitis are important to give effective rehabilitation management to patients, and manage them holistically to improve their functional performance.
