Effect of Intradialytic Isometric Exercise with or without Neuromuscular Electrical Stimulation on the Functional Capacity in Chronic Kidney Disease Patients

Abstract

One etiology of limited function in chronic kidney disease patients is muscle wasting, especially at the lower extremities. The American College for Sport Medicine (ACSM,2014) recommends strengthening exercises with moderate intensity (60-75% 1 RM) for chronic kidney disease patients, but it is hard to be done by stage V of chronic kidney disease as patients undergo hemodialysis. Isometric exercises and Neuromuscular electrical stimulation (NMES) are easy and can be applied intradialytically. Both of them facilitate muscle contraction and improve muscle strength so it can improve functional capacity. There are not many studies reporting the effect of intradialytic isometric exercise with or without NMES in chronic kidney disease patients. This study aimed to prove that intradialytic isometric exercise with or without NMES can improve functional capacity in chronic kidney disease patients. This experimental study had a pretest-posttest control group design. Twenty-five chronic kidney disease patients on hemodialysis were randomly divided into 3 groups: control group, isometric exercises group, and isometric exercises and NMES group, which received treatment twice a week, for 6 weeks. The subjects were examined by the five times sit to stand test (FTSTS) pre and post study. There was increased functional capacity (FTSTS) post study compared to pre study in the isometric exercises group (p=0.007) and also isometric exercises and NMES group (p=0.001), but not in the control group.
