Single-Visit Endodontic Therapy of a Tooth with a Periapical Abscess: A Case Report

Abstract

Background: Single-Visit Endodontic therapy is defined as a conservative, non-surgical treatment of an endodontically involved tooth, consisting of complete chemo mechanical cleansing, shaping and obturation of the root canal system in one visit. Eliminating bacteria from an infected root canal system and removing the inflamed pulp tissue and closing it with a biologically acceptable filling material is the main objective of performing root canal therapy. The recent advances in endodontic technology have attracted the dental practitioner as well as the endodontist to perform the root canal treatment in a single visit. Purpose: To report the outcome of single-visit endodontic therapy of a tooth with a periapical abscess. Case: A 72-year-old female patient came to the Department of Conservative Dentistry at Dental Hospital of Airlangga University with a complaint of a broken front tooth. The tooth was symptomatic. Clinical examination showed a fractured left maxillary lateral incisor (#22). Soft tissue surrounding the tooth was normal. Periapical radiography showed a periapical lesion. The patient’s medical history was non-contributory and single-visit root canal treatment was proposed. Case Management: Root canal treatment with a Crown-Down pressure-less technique was performed using pathfile and a protaper NextTM rotary instrument until X2. The canal was irrigated with 2.5% NaOCl prior to changing to the next sequence of endodontic rotary instruments. An endoactivator and photodynamic therapy (Fotosan®, 630 nm) were applied. Final irrigation was performed with 2.5 % NaOCl, 17% EDTA, 3% Chorhexidine and aquadest. The root canal was obturated with gutta percha cones and resin sealer (AH plus) using the single cone technique. The access opening was temporarily filled with cavit. The tooth was subsequently restored with a custom post and full porcelain crown on the next visit. After 1-month follow-up x-ray examination showed periapical tissue healing Discussion: With adherence to the up-to-date knowledge and strict endodontic treatment protocol, necrotic teeth with periapical abscesses can be effectively treated with single-visit endodontic treatment, thus benefiting patients with fewer dental visits.
