Abstract

Background: Discoloration of teeth is a common esthetic problem caused by intrinsic or extrinsic factors. Dental bleaching is considered a conservative and non-invasive treatment to change the color of discolored teeth. In-office bleaching procedures use a bleaching agent that contains a high percentage of H2O2 placed onto the teeth. In addition, light activated bleaching gels will accelerate the bleaching procedure and result in an efficacious bleaching treatment. Case: A 23-year-old female patient was referred to the Conservative Specialist Department of the Dental Hospital of Airlangga University with a complaint about discoloration of the upper and lower anterior teeth. This discoloration was caused by orthodontic treatment with a fixed appliance 1 year beforehand. Treatment: Pre-operative photos were taken and the initial shade color was C1. The teeth were polished with a brush and pumice. A gingival dam was applied to the gingival margins of the teeth. Two components of the bleaching gel (H2O2 40%) were mixed and applied using a syringe covering the buccal surface of all the anterior teeth and first premolar. Irradiation light was performed for two cycles during the treatment process. After that, bleaching gel was swabbed and washed away. The final shade determination was identified as W2. Desensitizing gel was applied to the teeth to reduce their sensitivity. The patient was recalled after one week for re-evaluation. Conclusion: In office bleaching using light activated hydrogen peroxide agent has been demonstrated to be an efficacious treatment that results in a significant change of the appearance due to tooth discoloration.
